Objective 5 consists of two key measures, both of which must be satisfied to receive credit:

3. Measure 1: Patient Access to View, Download, and Transmit Health Information
Requirements

Provide timely access for patients to view, download, and transmit their health information.

Ensure patient information is available for access using any application that meets API specifications at least 80% of the time.
Exclusion Criteria

No office visits during the reporting period.

Conducting 50% or more patient encounters in counties where 50% or more housing units lack 4 Mbps broadband availability.
Steps to Provide Patient Access

Access the Patient’s Registration Page:

Click on Register and search for the patient.

Set Up Patient Portal Credentials:

Click Patient Portal Password.
-
Either retain the default password or create a custom password.

Click Save.

Provide Login Information to the Patient:

Click Print Self.

A new browser tab will open with the patient's login information (URL, Clinic ID, User ID, and Password).

Print this document and provide it to the patient.

Instruct the Patient on Portal Access:

API Configuration for Patient Access

Steps to Provide Educational Resources

During an Office Visit:

Enter the patient’s ICD Codes.
-
Navigate to the Alert/Reminder Section at the top of the visit.

Click on any Educational Material that appears OR click Send All Education Link to Patient Portal.

From Medical History:
-
Go to Patient Home or the Current Office Visit.
-
Click the Education Section, then select Patient Education.
-
Click New Education.
-
Selecting and Printing Materials:

In the pop-up window, check off the materials you wish to provide.

Click Print:

The document will download to your computer.
-
Open and print the document to give to the patient.
-
Impact of Printing Educational Material:

Note: Even if materials are printed, they will be available on the patient portal, contributing to compliance.
