How do I Sign Up for Healthix Clinical Alerts

To sign up for Healthix clinical alerts:

1. Contact Healthix to enroll in the program.
2. Fill out and fax/email the Healthix form attached below to link with MDLand.
3. Have patients sign consent form from Patient Home > Doc & Lab > Forms (find form in Customize if not listed).
4. Upload signed consent form and update consent status in Patient Home > Registration > Consent > Healthix Consent.
